| Abstract | 「[1] 疾患概念」リウマチ熱(acute rheumatic fever: ARF)は, A群β溶血性レンサ球菌(group A streptococcus: GAS)による咽頭炎から, 約2〜4週後に続発する発熱・関節炎・心炎・小舞踏病・輪状紅斑・皮下結節などの症状を呈する非化膿性炎症である. 主に5〜15歳に発症し, 3歳未満や成人ではまれである. 全世界で毎年47万人が新規発症し, 約27.5万人がARFやリウマチ性心疾患(rheumatic heart disease: RHD)で死亡している. 平均的な発症頻度は19人/10万人である. 近年のわが国においては, 日本小児循環器学会の稀少疾患サーベイランスの報告で年間5〜10件くらいであり, 非常にまれである. 発症機序は, GAS抗原と宿主抗原の分子相同性による免疫学的交差反応であると考えられており, 主にリウマチ熱源性血清型菌株による咽頭炎後に発症する. |
