| Abstract | 「[1] 疾患概念」新生児の無呼吸発作は「20秒以上の呼吸停止」あるいは「20秒未満であっても徐脈またはチアノーゼを伴うもの」と定義される. 在胎37週未満の早産児にみられる未熟児無呼吸発作(apnea of prematurity: AOP)と, 疾患や病態による続発性無呼吸発作に大別される. AOPの頻度は在胎週数に逆相関する傾向がある. 在胎34〜35週で7%, 32〜33週で15%, 30〜31週で54%, 29週1,000g未満ではほぼ全例にみられる. 呼吸の調節は, 延髄に存在する呼吸中枢で管理されている. 呼吸中枢に存在する中枢性化学受容器と頸動脈小体に存在する末梢性化学受容器, 呼吸運動を感知する気道, 肺, 胸壁の機械受容器からの情報が呼吸中枢に集まり, その結果呼吸中枢からの刺激を呼吸筋に伝え, 呼吸運動を引き起こす. |
