| Abstract | 「Key Points」(1)生後2〜3週ごろからの非胆汁性の噴水状嘔吐をくり返すときは, 肥厚性幽門狭窄症を疑う. (2)腹部超音波検査で幽門筋の肥厚(4mm以上)や幽門管の延長(15mm)を認めれば確定診断となる. (3)以前より早期に診断されるようになり, 重度の脱水症や電解質異常に陥ることはまれだが, 嘔吐が続けば低クロール, 低カリウム性アルカローシスをきたす. (4)外科的治療(幽門筋切開術)と内科的治療(硫酸アトロピン療法)があり, それぞれのメリット・デメリットを把握し, 家族と十分相談したうえで治療方針を決定する. |
