| Abstract | 「はじめに」 脳機能モニター(cerebral functioning monitor: CFM)は, 1960年代のPriorとMaynardによる成人のICUでの使用報告がはじまりとされる. また, aEEG (amplitude-integrated electroencephalography)は比較的シンプルなパターン認識で脳の電気的背景活動の, 長期間の変化や傾向を評価できるモニター方法の一つであり, 新生児領域においては1970年代後半から諸外国で徐々に使用され始め, 脳低温療法多施設共同試験が米国, 欧州で次々と行われ, その導入基準の判定に採用されたことも, その普及に拍車をかけたと思われる. 新生児仮死の予後判定が生後6時間以内のaEEGと相関がみられ, 非常に早期の予測が可能であり, 広く使用されはじめている. 我が国でも2015年版新生児蘇生法(NCPR)ガイドライン(NCPR2015)で新生児蘇生法の一環として低体温療法が推奨され, その導入にaEEGが参考になるとされ, ずいぶんと広まった. |
