| Abstract | 「はじめに」新生児の呼吸管理においてステロイドは効果が高い薬剤である. 特に早産児の呼吸管理においては, 慢性肺疾患(chronic lung disease: CLD)の予防, 治療に重要な役割を担う. しかしステロイドは, 短期的には消化管穿孔や壊死性腸炎, 易感染性, 高血糖など, 長期的には脳性麻痺といった重篤な副作用のリスクが伴う薬剤であることも指摘されている. 使用するステロイドの種類, 投与目的(予防・治療), 投与量, 投与経路については, これまで多くの検討がなされているが, 一定のコンセンサスは得られていないのが現状である. 本稿ではCLDに対する出生後のステロイドの投与について, 過去の報告を提示しつつ検討していきたい. 「ステロイドの全身投与について」新生児領域において全身投与でよく使用されるステロイドは, ハイドロコルチゾン(HDC)とデキサメタゾン(DEX)である. |
