Abstract We examined whether home-visit rehabilitation, in which the goal of "Living like that person" was set using Management tool for daily life performance (MTDLP), had an effect on the physical and mental functions, life functions, and quality of life of community-dwelling older adults certified as long-term care residents. The results of the 6-month intervention showed significant improvements in physical function, life function, and quality of life at 3 and 6 months after the start of the intervention. The results of propensity score matching between the MTDLP group and the control group using historical data showed that the MTDLP group exhibited significant improvement in instrumental activities of daily living at both 3 and 6 months after the start of the intervention. The results indicate that home-visit rehabilitation using MTDLP could promote "activity" and "participation" of older adults in need of long-term care who live in the community.
