Abstract | 「要旨」急性呼吸促迫症候群は何らかの侵襲の後に, 肺に炎症が生じることで微小血管の透過性が亢進し肺水腫が起こる病態である. その結果, 重症呼吸不全となりICUで人工呼吸管理が必要となることも多い. しかし, 人工呼吸器管理そのものが人工呼吸器関連肺障害と呼ばれる病態を起こし得る. それを防ぐために肺保護換気戦略と呼ばれる, 低用量換気と肺胞の虚脱を防止する適切な管理が必要となる. 死亡率が高いと判断された場合, 体外式膜型人工肺の適応が考慮される. 人工肺により酸素付加, 二酸化炭素除去を行うことで肺をより休ませることができるが, 適切な管理は容易ではなく, 習熟した医療チームで行われるべきである. |
