| Abstract | 「要旨」下肢深部静脈血栓症(DVT)の典型的な症状は, 急性に発現する下肢の腫脹, 疼痛, 暗赤色への色調変化, 側副血行路の発達である. しかし, 急性例のおよそ50%では, 症状や所見が認められないともされる. 臨床症状からDVTが疑われれば, Wellsスコアで評価し, 低・中確率であればD-dimer検査を実施, 高確率であればD-dimer検査をスキップし, 次に全下肢静脈超音波検査をBモードで実施, さらに血流描出ができる方法(カラードプラ法など)を適宜併用する. 超音波検査で診断がつかない場合, 造影剤アレルギーや腎機能障害がなければ造影CTなどで診断する. これが現在の検査手順のスタンダードである. |
