Abstract 「要旨」 超高齢社会に突入したわが国では, 高齢者が手術を受ける機会が増加している. 術後せん妄は, 高齢者に多く予後とも関連する術後合併症である. その病態には, ミクログリアの過剰活性に伴う脳内神経炎症が関与する. ミクログリアは老化により炎症反応性が亢進することから高齢者では脆弱性が高まる. 術前リハビリテーション治療は, 老化ミクログリアの炎症反応性を軽減することで術後せん妄を予防できることが動物研究で示され, 臨床研究でも有用性を示唆する報告がなされている. 本稿では, 術後せん妄に焦点を当て, 術前リハビリテーション治療が高齢手術患者の回復の質を高める有用な方策となり得るか, 現時点での課題と今後の展望も合わせて解説したい.
