| Abstract | 「要旨」人工股関節全置換術における術後脱臼は, 患者にも医療者にも大きなフラストレーションとなることが多い. 脱臼をきたす要因としては, インピンジメントや不適切な軟部組織バランスが挙げられ, インプラントの設置アライメント不良や, 設置位置不良, 不適切なオフセット再建, 脊椎変形・脊椎可動性不良, 患者の理解不足, 感染, 摩耗, その他といったものがインピンジメントや軟部組織バランスの破綻を引き起こす. それらのメカニズムを理解することは重要である. また, 脱臼を制動する軟部組織の強度は手術アプローチによって異なるため, 各アプローチの特徴を十分理解しつつ, 術後のリハビリテーション治療と患者の生活指導を行う必要がある. |
