Abstract | 「要旨」COVID-19後遺症とは, 感染後数カ月経過してもなお種々の症状が持続するものであり, 最も頻度の高い倦怠感は感染者の2〜4割に認められる. 治療は確立しておらず種々の症状の構成要素へそれぞれ対処するしかないが, 特に心肺機能障害としては肺拡散能の低下および心筋炎の存在に注意が必要である. 急性期に軽症であってもこれらは残存している可能性がある. 認知機能障害としてはブレインフォグが特徴的であり, 当科では反復性経頭蓋磁気刺激(rTMS)を用いた治療的介入を行っている. リハビリテーション治療や生活において身体・認知負荷をかける場合, 負荷により症状が増悪するpost-exertional malaise(PEM)という現象に注意し, 段階的な社会復帰をめざさなければならない. |
