| Abstract | 「要旨」 絞扼性神経障害は, 末梢神経が生理的狭窄部において生じる神経障害の総称である. その病態は物理的圧迫だけでなく, 牽引, 血流障害, 摩擦など多岐にわたる. 外科的に神経障害因子を除去することが最も有効な治療法であるが, その前に非侵襲的に末梢神経の環境改善を行い, 神経再生をもたらすことで症状の改善が期待できる. これらの保存療法で改善が得られない場合は, 神経剥離術が適応となる. しかし, 特に運動障害が長引いて筋萎縮を生じると神経剥離術では機能改善が得られず, 腱移行術などの再建手術を要することも少なくない. リハビリテーション科医は保存療法のみならず手術療法にも精通し, 適切な治療適応を選択することが重要である. |
