Abstract | 「はじめに」 骨盤底とは, 膀胱・子宮・直腸といった骨盤内臓器を支える筋・筋膜・靱帯の総称であり, 骨盤内臓器を適切な位置に支持する機能と, 尿道括約筋や肛門括約筋と協調して尿道や肛門を閉鎖する機能を担う. そのため, 骨盤底のどこかに障害が起きると, 骨盤臓器脱, 便失禁, 尿失禁などの症状が引き起こされる. 女性の骨盤底障害の主な原因は妊娠・出産であり, 経腟分娩時に骨盤底筋群は普段の筋の長さより2〜3倍も伸展し, 微小な筋の断裂や離開が起きる. そのため, 女性の骨盤臓器脱, 尿失禁の発症リスクは分娩回数とともに高くなり, 30代以降は尿失禁有病率が20%を超えるようになる. 男性では, 骨盤底障害の代表的な原因は前立腺がんに対する前立腺全摘除術である. 前立腺全摘除術による尿道長の短縮や神経損傷をきっかけに, 術後は約9割の男性が尿失禁を経験し, 1割は術後1年以降も尿失禁が持続する. |
