| Abstract | 「要旨」身体的フレイルは心血管疾患, 特に心不全を有する高齢者において高率に併存し, QOLや予後と強く関連する. フレイルと心血管疾患の病態には相互関連性が示唆されており, フレイル対策はQOL向上や介護予防に留まらず, 疾病の重症化予防にも寄与する可能性がある. サルコペニアは身体的フレイルの中核となる因子であるが, 心不全の病態コントロールが不良な症例では二次性サルコペニアを想定した包括的心臓リハビリテーションが対策のカギとなる. 本稿では, 心血管疾患におけるフレイルの臨床疫学, フレイルの多面的な評価と心臓リハビリテーションにおける位置づけ, 介入方策について, 現状のエビデンスに基づいて整理した. |
