| Abstract | 「要旨」脳卒中はわが国において, 要支援・要介護の主原因の第2位であり, 死因の第4位を占める. 脳卒中リハビリテーション医学・医療の歴史は古いが, 最近30年で大きく変化した. 2000年に回復期リハビリテーション病棟, 介護保険制度の導入以降, 回復期, 生活期という用語が徐々に定着した. 同時期より, 脳卒中リハビリテーション医学に関するエビデンスが増え, 脳卒中リハビリテーション医療の保険診療が大きく変遷してきた. また2004年に脳卒中治療ガイドラインが発行され, 改訂が行われてきた. 早期リハビリテーション, 訓練量や頻度を増やすこと, 装具療法, ボツリヌス療法, CI療法(constraint-induced movement therapy), 機能的電気刺激, 多職種連携, 患者・家族教育は推奨度が高い治療法として定着した印象がある. 最近のトピックスとしては, 脳卒中患者の高齢化とフレイル・サルコペニアの合併, 超早期リハビリテーション, ロボットリハビリテーションなどが挙げられる. 保険診療において, 2018年より早期離床・リハビリテーション加算, 2020年より運動量増加機器加算が可能となったが, これらに関連した課題も残されている. |
