| Abstract | 「要旨:」急性肝不全は代謝中心臓器である肝臓の急激な機能喪失を契機として, 複数の臓器不全が併発する短期予後が極めて不良な症候群である. 急性肝不全に対する内科的治療では, 成因に対する治療, 肝障害進展を抑制するための治療, そして合併する多臓器不全に対する治療など, 多角的な集中治療が必要となる. 本邦では血漿交換と血液濾過透析を組み合わせた治療が人工肝補助として急性肝不全に対する内科的治療の主体をなしてきたが, 近年では高効率の血液濾過透析が肝性脳症の覚醒率の高さから注目を集めている. 現在, 肝不全に陥った肝臓を積極的に再生させ得る内科的手段はないため, 救命困難と判断された場合は肝臓移植が必要となる. |
