| Abstract | 「要旨」 : 総胆管結石は無症状であっても治療対象であり, 存在が疑われた場合にはUS, CT, MRI (MRCP) , EUSなどの画像検査を施設や症例の状況に応じて選択し, 診断を進める, 治療は現在, ESTを主とした内視鏡治療が多くの施設において第一選択とされており, 胆嚢結石合併例に対しては内視鏡的総胆管結石除去術ののち腹腔鏡下胆嚢摘出術を行う方針が主流であるが, 外科的一期的手術 (胆嚢摘出+総胆管結石除去) を行っている施設もある. また, 術後再建腸管例や大結石, 合流部胆石など従来内視鏡治療が困難とされてきた症例に対しても, 近年, バルーン内視鏡, 経口胆道鏡, EUSなどを用いた手技の開発によって治療が可能となり, 内視鏡治療の適応範囲が広がっている. |
