| Abstract | 「要旨」抗リン脂質抗体(aPL)は, 凝固時間法によるループスアンチコアグラント(LA)と免疫学的な測定による抗カルジオリピン抗体(aCL)やβ2グリコプロテインI依存性抗カルジオリピン抗体(aCL/β2GPI)および, aCL IgG, IgMとaβ2GPI IgG, IgMの4種を同時測定する抗リン脂質抗体パネル検査が保険診療において測定可能である. 海外の標準化の進展とともに国内においても測定標準化に向けた検討が進められている. aPLの検出は, ISTH/SSCの推奨法によりLAが陽性であること, aCL/β2GPIまたはaβ2GPIのIgGまたはIgM抗体が陽性(健常者の99%タイル値より高値)であること, また, これら3種のaPLが同時に陽性の場合, 血栓症などの発症リスクが高いことから, 確実にaPLを検出するためにも全ての測定を実施することが重要である. |
