Abstract 「要旨」【目的】自閉スペクトラム症(autism spectrum disorder;ASD)患者における幼児期の感覚処理障害(sensory processing disorder;SPD)とASD重症度・対人コミュニケーション障害との関係を明らかにする. 【方法】ASD重症度・対人コミュニケーション障害は日本の質問紙である親面接式自閉スペクトラム症評定尺度テキスト改訂版(PARS-TR;PARS), SPD重症度は日本版感覚プロファイル(Sensory Profile(TM);SP)にて評価した. PARS短縮版とSPの相関性をSpearmanの順位相関係数を調べることにより検討した. 【結果】ASD患児26人における, PARS短縮版とSP総得点は中程度の正の相関を認めた. 感覚の四象限(感覚過敏・感覚回避・低登録(鈍麻)・感覚探求)では, 感覚過敏, 感覚回避, 感覚探求の三象限で正の相関を認めた. 感覚モダリティ別の評価では, 聴覚, 視覚, 前庭覚, 触覚, 複合感覚と複数のモダリティで正の相関を認め, 対人コミュニケーションに限った得点でも同様であった. 【結論】ASD患児の幼児期のSPD重症度はPARS短縮版と正の相関がある. PARS短縮版高得点の児では, SPD併存の可能性も念頭に置く必要がある.

Abstract Objective: The present study investigated the relationship between the severity of sensory processing disorder (SPD) and both the over-all severity of autism spectrum disorder (ASD) and the severity of social communication difficulties in early childhood among individuals with ASD. Methods: The severity of ASD and social communication difficulties was assessed using the short version of the Japanese questionnaire, the Parent-interview ASD Rating Scale-Text Revision (PARS-TR, PARS), while the severity of SPD in patients with ASD was assessed using the Japanese version of the Sensory Profile(TM)(SP). The presence or absence of a relationship between the scores of the short version of PARS and SP was investigated using Spearman's rank correlation coefficient. Results: Twenty-six children with ASD were analyzed. A correlation was observed between the scores of the short version of PARS and the total scores of SP. Regarding the four quadrants of sensory sensitivity (Poor Registration, Sensation Seeking, Sensitivity to Stimuli, and Sensation Avoiding), correlations were found in the three quadrants : Sensation Seeking, Sensitivity to Stimuli, and Sensation Avoiding. In the evaluation based on sensory modalities, correlations were observed in the Auditory, Visual, Vestibular, Tactile, and Multisensory modalities. Therefore, correlations were noted across multiple sensory modalities. Similar results were also observed when the scores were limited to social communication difficulties in the short version of PARS. Conclusions: The severity of SPD in early childhood among individuals with ASD correlated with both the overall severity of ASD and the severity of social communication difficulties, as assessed by the short version of PARS. For children with high scores on the short version of PARS, it is necessary to consider the possibility of comorbid SPD and to provide medical care accordingly.
