Abstract	福山市では福山ネウボラ事業として, 妊娠, 出産, 子育てに関し, 切れ目のない支援を行うために, 医療・保健・福祉等の相談体制を再構築し, 安心して子育てができる環境を整備している. 社会的・精神的ハイリスク妊婦に対して医療機関より行政に情報提供を行い, 産前・産後に保健師が介入してサポートを行っている. その中で産後の育児が著しく困難と予想される症例では, 妊娠中から母子分離を含めて検討する. つまり育児困難の可能性のある妊婦に関して, 妊娠早期より医療機関及び行政によるサポート体制の構築は母児安全のために必要である. 当院では, 2018年4月より社会的ハイリスク妊婦の中でも育児困難症例に対して, 医療機関及び行政機関による多職種カンファレンスを開始した. 各機関からの病状や社会状況等の現状報告及び課題を上げ, 解決策を検討することにより, 妊娠期から子育て期への切れ目のない充実した支援のきっかけになることが期待できる. 今回当院では妊娠中から妊産婦へのサポート体制を構築により, 特別養子縁組や育児サポートへ繋がった症例を経験したため報告する.

Abstract	Pregnant women with high social risks need the continuous support of medical and social institutions throughout pregnancy and later for appropriate childcare. Our hospital and Fukuyama city has introduced an interprofessional conference since April 2018 for the benefit of social high-risk pregnant women, particularly those experiencing difficulty with childcare. The Finnish neuvola system (an important component of the Finnish welfare model) has been drawing marked attention in Japan. This model focuses on providing support to expectant mothers, offering infant and childcare services, and promoting the welfare of the whole family. An interdisciplinary approach to social high risk women is an important feature of the Fukuyaka-Neuvola system. We report 3 cases in which an interprofessional conference aided in developing a support system for social high-risk pregnant women. We anticipate that this interdisciplinary approach will lead to better mother-child safety and prevent or minimize child abuse by offering the necessary support to such women.
