| Abstract | 「慢性心不全の薬物療法」 慢性心不全の薬物療法は, 左室駆出率(LVEF)の値に基づいて決定される. かかりつけ医は問診や身体所見, あるいはBNPやNT-proBNPの測定結果から, 心不全を疑った場合には, 速やかに, 循環器専門医に心臓超音波検査等の精査を依頼することが肝要である. 専門医は検査結果から, 原疾患の鑑別診断とその治療, LVEFに基づいた標準的薬物療法を開始する. ただし, うっ血症状に対しては, LVEFの如何に関わらず利尿薬が推奨される. また, 予後改善を目的としたSGLT2阻害薬の使用もLVEFの値に関わらず推奨されている. 慢性心不全の薬物療法のエビデンスは, 心不全ステージ分類のステージB(器質的心疾患は認めるが, 心不全症状はない), あるいはステージC(器質的心疾患も心不全症状も認める)を中心に構築されていることを念頭に置く必要がある. 心不全症状を改善させることは, QOL(生活の質)を担保する上で重要である. |
