| Abstract | 「はじめに」 本稿の内容は第69回日本アレルギー学会学術大会の教育講演に基づくものだが, タイトルの「アナフィラキシー Up-to-date」に鑑みて, COVID-19禍に関連した情報も追記したい. 「アナフィラキシーの診断」 アナフィラキシーとは, 「アレルゲン等の侵入により, 複数臓器に全身性にアレルギー症状が惹起され, 生命に危機を与え得る過敏反応」であり, 血圧低下や意識障害を伴う場合をアナフィラキシーショックと呼ぶ. アナフィラキシーの多くはIgEが関与する免疫学的機序により発生し, 最も多くみられるアレルゲンは, 食物摂取, 刺咬昆虫(ハチ, アリ)の毒, 医薬品である. 発症時は時間的余裕がないため, バイオマーカーではなく臨床所見により診断されるが, そのポイントは2つに絞られる. 一つは, 急速に出現する複数臓器の症状(皮膚・粘膜, 呼吸器, 循環器, 消化器症状などだが, 蕁麻疹, 皮膚紅潮と掻痒, 口唇・舌・口蓋垂の腫脹など皮膚・粘膜症状の頻度が圧倒的に高い)であり, 他の一つは症状が現れる直前の「その患者にとってのアレルゲン」あるいは「一般的にアレルゲンになり易いもの」への曝露状況である. |
