Abstract | 「はじめに」 アトピー性皮膚炎(Atopic Dermatitis ; AD)は皮膚炎の増悪と寛解を繰り返す慢性皮膚疾患で, 患者ごとに多彩な臨床像・背景をもつ多因子疾患である. 病態や症状の増悪に関連する因子は明らかになったものもあるが, 病態の全容は依然解明されておらず, 根治方法も確立していない. ADは12歳未満の小児の20%に見られ, 成人の7-10%が罹病しているとされている. 本邦におけるADの患者数は数十万人に及ぶと考えられ, AD患者は繰り返す皮膚炎によりQOLが低下し, また, AD患者は加齢とともに食物アレルギー・喘息・花粉症等のアレルギー疾患に罹患する傾向があることが知られ, ADの克服は依然大きなテーマである. 「アトピー性皮膚炎の病態に関連する因子」 ADの病態に重要な因子としては, フィラグリン遺伝子の変異などによる皮膚バリア機能の障害とTh2型サイトカインを誘導する免疫異常が以前より知られていたが, 近年次世代シーケンサーを用いたマイクロバイオーム研究により明らかとなった皮膚細菌叢の多様性の低下(dysbiosis)が注目されている. |
