| Abstract | 「はじめに」自己炎症性疾患は, 自己免疫疾患とは異なる機序により発症し, 自己抗体を持たないなどの特徴をもつ炎症性疾患として, 1999年にKastnerらによって提唱された疾患概念である. 古くから知られている家族性地中海熱やTNF受容体関連周期性症候群などの周期性発熱症候群だけでなく, 次々と新たな自己炎症性疾患の存在が明らかとなり, 血管炎や脂肪織炎などを呈するインターフェロノパチーや, ベーチェット病様症状や肉芽腫性炎症などを呈するNF-κB/TNF活性異常症など, 多様な病態の自己炎症性疾患が知られるようになってきている. 本稿では, アレルギー専門医が持つべき基本知識として, 自己炎症性疾患の分類, 代表的な自己炎症性疾患である周期性発熱症候群, そして自己炎症性疾患の診断のための遺伝学的検査について概説する. |
