Abstract 「はじめに」21世紀は気管支喘息やアトピー性皮膚炎の有病率は増加が一段落した感があるが, 食物アレルギーに関しては, 先進国を中心にEpidemicの観を呈している. しかし, 乳幼児に多いIgE依存性の食物アレルギーに関しては, 二重抗原曝露仮説を実証する介入研究が相次いで発表され, 予防が現実的な視野に入ってきた. ガイドラインは網羅的な文献検索をベースにして作成されているため, 膨大な文献と様々なエビデンスレベルの情報が記載されているが, 本稿では, 高いエビデンス水準の臨床研究を中心に, 実臨床の現場で役立つトピックスに絞って解説する. 「食物アレルギーのリスク因子」「(1) アトピー性皮膚炎」食物アレルギー診療ガイドライン2021には, 「小児期の食物アレルギー発症リスクに影響する因子として, 家族歴, 特定の遺伝子, 皮膚バリア機能, 日光・ビタミンDなどが報告されているが, 中でもアトピー性皮膚炎が重要である」と記載されている.
