| Abstract | 「はじめに」「運動器超音波検査に必要な臨床解剖」の第2回として, 肘関節について概説する. 肘関節における超音波画像といえば, 下記に述べるような疾患でのニーズが高いと想定される. 外側であれば上腕骨外側上顆炎における浅層伸筋群や橈骨神経, 不安定症における外側々副靱帯 (lateral collateral ligament : LCL) や橈骨輪状靱帯, 離断性骨軟骨炎における小頭軟骨面の観察が思い浮かぶ. 内側であれば内側々副靱帯 (medial collateral ligament : MCL) 損傷におけるMCL自体や回内屈筋群, 尺骨神経障害における尺骨神経と尺側手根屈筋などを同定する必要がある. 肘関節は非常に狭い領域の中に様々な筋腱や靱帯が混在していることになるが, それぞれの筋腱と靱帯との境界はどうなっているのか, 最深層にある関節包はそれらの構造に対して, どのような層関係になっているのか, など超音波画像にうつりこむ構造を深く理解しようとするほど, 疑問は増えていくのではなかろうか. |
