Abstract	「要旨」我が国の高齢者人口は増加の一途をたどり, 75歳以上人口は2054年まで増加傾向が続くと見込まれている. その中でも『地域包括ケアシステム』の推進に代表されるように, 高齢患者であっても精度の高い転帰予測と自宅退院に向けた効率的な介入が必要とされている. また, 機能維持の観点からも退院後の閉じこもり予防に対する配慮も求められている. 今回, 心原性脳塞栓症により運動性失語と廃用を来たした超高齢患者A氏を回復期リハビリテーション病棟にて担当した. 失語症状や身体機能の低下から自信を喪失したA氏に対して, 訓練内容の段階付けや正のフィードバック等を通して精神機能面に配慮した介入を行い, 在宅復帰の達成と閉じこもり予防として病前も使用していた短時間通所リハビリテーションの再開を行う事ができた. 介入を通して, 本人にとっての"生活"の意味や地方の弱みと強みに関して気づきを得る事ができた.

Abstract	[Abstract] The elderly population in Japan is steadily increasing, and the population aged 75 and over is expected to continue increasing until 2054. Even under such circumstances, as represented by the promotion of the "Community-based Integrated Care System", accurate outcome prediction and efficient intervention for discharging from hospital to home are required even for elderly patients. In addition, from the viewpoint of maintaining function, consideration is required for prevention of homebound after discharge. This time, I was in charge of Ms. A, who developed cardiogenic embolism and motor aphasia and disuse syndrome, in the Kaifukuki Rehabilitation Ward. Ms. A had lost self-confidence due to aphasia and decreased physical function, but she was able to be discharged to home and resume short-term rehabilitation for the purpose of preventing homebound after discharge by intervention with consideration for mental function. Through the intervention, I was able becoming to gain an awareness of the meaning of "life after leaving hospital" for the patient and the advantages and disadvantages of rural area.
