| Abstract | 「要旨」今回, 心原性脳塞栓症を再発し療養型病院からA病院ICUに入院した症例を担当した. ICU入院という家族と患者が離される状況に加えて, 新型コロナウィルスの感染拡大による面会制限など感染対策への厳正さを増す中での作業療法の実践となった. 症例の家族からは「近くにいるのに会いに行けない, やっと面会が出来ても変わり果てた本人の姿を見ると辛く, 何もしてあげられないと涙が止まらなくなる」と表出があった. そこで, 家族の承諾を得てリハビリ時の症例の様子や表情を写真に収め, 作業療法士の言葉を添えたお便りを家族に渡すことを実践した. そして, 家族が医療チームの一員であるという感覚を持ち続けられるよう, お便りを継続的に渡し, 家族を含むチーム内での情報共有を行った. その結果, 徐々に面会時の家族は症例を目の前に涙する時間の中にも, お便りを見ながら症例の姿をICU看護師と振り返り, 症例に触れ, 語りかける時間が生まれる変化が見られた. |

| Abstract | [Abstract] This time, I was in charge of a patient who was admitted to our ICU from a convalescent hospital due to a recurrence of cardiogenic cerebral embolism. The patient's family complained that they could not visit him even though he was close by, that it was difficult to see any changes even if they could finally visit him, and that they could not do anything for him and could not stop crying. Therefore, with the family's consent, we took pictures of the patient's condition and facial expressions during rehabilitation and gave them a letter with advice from the occupational therapist. The letter was immediately handed over to the family so that they could continuously feel that they were part of the medical team, and the information was shared within the team including the family. Instead of grieving, more time was spent looking at the letters, talking to the patients, comforting the patients, and reflecting on the patients' condition with the ICU nurses. |
