Abstract 「抄録」認知症高齢者を対象としたelderspeakに関する研究の結果と研究上の課題を明らかにすることを目的とし, 文献検討を行った. 医中誌Web, CiNii, Scopus, PubMed, CINAHLを用い, elderspeakをキーワードとして検索した. 抽出された9論文について整理し, データ収集方法, 結果からelderspeakの課題を検討した. elderspeakは, 急性期病院や介護施設の日常生活ケア時に使用されており, 介護拒否行動と関連していた. その他, せん妄や日常生活動作の低下などとも関連していた. 参加観察法は, ケアスタッフと認知症高齢者の相互作用を確認するために適切なデータ収集方法であるが, 認知症高齢者の緊張への配慮が必要である. ケアスタッフはelderspeakの使用を不適切と考えているが, 実際の行動とは一致していなかった. わが国においてもelderspeakに関する研究の蓄積が必要と考える.

Abstract This study aims to clarify the actual use of elderspeak in communication with older adults with dementia and to explore related research issues. Using "elderspeak" as a keyword, relevant articles were identified on PubMed, Scopus, CINAHL, the Ichushi (Japan Medical Abstract Society), CiNii. A total of 9 articles were selected for analysis and review. The findings indicate that elderspeak is commonly used during daily care in acute care hospitals and nursing homes. Its use has been associated with care-resistant behaviors among individuals with dementia, as well as with delirium and lower levels of activities of daily living (ADL). The participant observation method required preliminary preparation to allow older adults to become accustomed to the presence of observers and to ensure sufficient research time. Ethical and methodological considerations for older people with dementia are crucial when conducting participant observation. Although elderspeak was widely used, staff perceived it as an inappropriate form of communication, revealing an inconsistency between their attitudes and actual practices. Further research is necessary to clarify the current state of elderspeak use in Japan.
