| Abstract | 「要旨」【目的】一般病棟で働いている中堅レベル看護師「緩和ケア」に関する成長・発達のプロセスを分析した. 【方法】A都道府県がん診療連携拠点病院の一般病棟でがん患者に関わる中堅レベルの看護師8名にインタビューし, 修正版グラウンデッドセオリー・アプローチ(Modified Grounded Theory Approach:M-GTA)にて分析を行なった. 【結果】7のカテゴリーと, 26の概念が生成された. 【結果と考察】一般病棟の中堅レベル看護師の緩和ケアは「一般病棟での緩和ケアの困難感を実感」し「患者の真の苦痛を理解できたという想い」に変容し「緩和ケアを提供できたと喜びを感じられること」さらに「自分の成長を実感できること」と段階的に経過をたどっていた. 「患者の真の苦痛を理解できたという想い」は, プロセスの中心となっていた. <全人的苦痛を抱えた患者に寄り添うこと>で「患者の真の苦痛を理解できたという想い」が生じ, <患者の真の苦痛を緩和することに取り組むこと>で「緩和ケア」に関する成長・発達のはじまりであると考えられた. |

| Abstract | [Abstract] The purpose of this study was to clarify the growth and development process related to "palliative care" of mid-career nurses working in a general ward. We conducted semi-structured interviews with 8 mid-career nurses currently involved with cancer patients in the general ward of a Designated Regional Cancer Hospital in A Prefecture. The verbatim transcript was created from the interviews, and analyzed using the Modified Grounded Theory Approach (M-GTA). The analysis generated 7 categories and 26 concepts. The categories are; [Realizing the difficulty in delivering palliative care in the general ward], [Feeling to have understood well about patient's real suffering], [Willingness to acquire skills for palliative care], [Awareness that multidisciplinary team care is necessary to meet patients' needs], [Learning from nurses who practice palliative care as role models], [Joy of providing palliative care to patients], and [Feeling of self-growth]. It was found that mid-career nurses can ease their dilemma in delivering palliative care, by enhancing their expertise through interventions from their senior nurses or palliative care team, and getting "a feeling to have understood well about patient's real suffering". In addition, it was suggested that the process of growth and development related to palliative care can be utilized in the clinical settings when guidance is provided taking account into the problems and issues of palliative care education and effective educational contents. |
