Abstract	「和文抄録」本研究の目的は, 患者のがん治療の時期の違いによる看護師のもつジレンマの特徴について検討を行うことである. 未成年の子どもをもつがん患者の看護に携わった経験をもつ看護師を対象とし, インタビューを行った. 本研究では, 「看護師として感じていたジレンマ, もっとこうすればよかったと感じること」を報告する. 対象者は看護師15名, 年齢は27歳から58歳で, 平均は41.4±9.93歳であった. 有している資格は, がん看護専門看護師が4名, がん化学療法認定看護師が1名, 緩和ケア認定看護師が1名であった. テキストマイニングのためのソフトウェアであるKH Coderを用いて, ジレンマについて述べられた内容に関して計量テキスト分析を行った. 対応分析では, 治療期は「計画」「説明」「家族」等, 再発・進行期は「緩和」「ケア」等, 終末期は「面会」「亡くなる」等が特徴語として得られた. 時期ごとの看護師のジレンマの特徴を共起ネットワークで検討した. 治療期は, 【医療者による母親や子ども, 家族に理解しやすい説明の模索】【母親の意思を尊重した上での, 子どもに病状説明する際の支援】等6個のカテゴリー, 再発・進行期は, 【母親の「緩和」の認識の誤解により必要な緩和ケアを受けてもらうことの難しさ】【家族を思い治療に励む母親に寄り添うケアと, 感染予防対策との間の模索】等4個のカテゴリー, 終末期は, 【子どもへの告知に対する母親の意思の尊重と看護師の葛藤】【医師からの情報と母親の希望をふまえた, 亡くなるまでの限られた入院時間でできる看護の模索】等5個のカテゴリーが得られた. 看護師が感じるジレンマは, 患者と家族にとって「自分はどうあるべきか」「どう行動すべきか」という, それぞれの看護師の看護観と倫理観が影響していると考えられた.

Abstract	[SUMMARY] This study examined the characteristics of dilemmas experienced by nurses across different phases of cancer treatment in patients with underage children. Semi-structured interviews were conducted with fifteen nurses aged 27-58, who had experience caring for mothers with cancer raising minor children. This study explored the dilemmas that the nurses faced, including reflections such as "I wish I had done things differently." Quantitative text analysis was performed using KH Coder. Co-occurrence network analysis was used to identify the features of dilemmas in each treatment phase. In the treatment phase, six categories were identified, including "struggling to provide understandable explanations to mothers, children, and families." Four categories were identified in the recurrence/progression phase, including " difficulty in providing necessary palliative care due to mothers' misunderstanding of palliative care." In the terminal phase, five categories were ascertained, including "conflict between respecting the mother's wish not to disclose the prognosis to her child and the nurse's own ethical stance." The findings suggest that the nurses' dilemmas are shaped by their individual nursing philosophies and ethical values. These dilemmas often stem from the internal struggle over how they should maintain professional integrity while supporting both the patient and her family.
