| Abstract | 当院の痛みセンターでは看護師, 薬剤師, 精神科医, ペインクリニック医の集学的多職種診療を行っており, 最初の診察者である看護師が患者との信頼関係構築と心理社会的要因の把握という重要な役割を担っている. 看護師は患者の話を丁寧に聴くが, 単なる聞き役にとどまらず, 看護師自身の気づきも伝える双方向の対話を重視している. 事例は, 10年以上持続する腰背部痛が主訴の70歳代女性. 地方へ転居後に体重が20kg増加し, 痛みが増強したことに対し, 看護師は知人が不在で孤独から過食に至った気持ちに寄り添った言葉をかけた. すると患者は多数の受診歴の中で初めてギャンブル依存について語り, さらに患者自身が孤独感からのギャンブル依存が痛みの原因だと気付き, 長時間の同一姿勢と体重増加が原因の二次性慢性筋骨格痛と診断された. 看護師の対話がケアになり信頼関係を構築し, 女性にとって話づらい内容の自己開示を促し, 心理社会的要因の発見が慢性疼痛の治療につながっていた. |

| Abstract | The Pain Center of our hospital provides multidisciplinary medical care. The nurse's consultation is responsible for building trust with the patient and identifying psychosocial factors. The case was a woman in her 70s, pain in the lower back that persisted for more than 10 years without any cause. Seven years ago, after moving to a local area, she gained 20 kg of weight and her pain increased, and the nurse's empathetic words made her realize a loneliness that she couldn't tell anyone else about. After that, she started talking for the first time about playing "Pachinko" (popular Japanese gaming activity) for a long time, which she had never talked about to doctors. The cause of the pain was found to be weight gain and prolonged posture, and the diagnosis was chronic secondary musculoskeletal pain. A relationship of trust was built through careful listening by the nurse, and dialogue became care. The nurse's care, which focused on the patient's feelings, led to the discovery of psychosocial factors. Nurses are expected to play an active role in chronic pain medicine in the future. |
