Abstract 「要旨」本研究は, 先行研究から認知症高齢者への疼痛評価について検討し, 今後の研究課題を明らかにすることを目的とした. 医中誌Webを使用し「認知症」, 「疼痛」, 「評価」に関連するキーワードにて検索を行い, 11件の文献を分析対象とした. 分析の結果, 認知症高齢者に対して, 自己報告や観察及び家族・多職種からの情報に基づいた疼痛評価が実施されていた. 自己報告に基づく評価と観察に基づく評価は併用し, 対象の認知機能を見極めたうえで適した方法を用いる必要がある. また, 認知症高齢者に関わる多職種が専門性を活かし, それぞれ疼痛評価を行うことが求められる. 加えて, 疼痛評価スケールの使用により, 背景の異なる多職種の評価指標を統一することが望ましいであろう. 今後は, 看護師以外の職種が実施する疼痛評価の実態の調査や, 疼痛評価に評価スケールが使用されない原因の調査及び疼痛評価者への教育が必要となると考えられた.

Abstract [Abstract] The purpose of this study was to review previously published studies to examine the pain assessment for older people with dementia in Japan, and to highlight directions for future research. A paper search via Ichushi-Web was carried out with keywords such as "dementia," "pain," and "assessment," and 11 studies were selected for analysis. The results indicated that current pain assessment in older people with dementia are based on self-reporting, observation, and collecting information from family members and multiple professionals (doctors, nurses, rehabilitation staff, and caregivers). Self-reporting and observation should be conducted in conjunction for pain assessment, and appropriate methods should be used after assessing cognitive function. Additionally, each professional who supports older people with dementia needs to assess pain on the basis of their own expertise. It may be beneficial to unify assessment indices across multiple professionals using common pain assessment scales. In the future, it was considered necessary to survey the state of pain assessment conducted by multiple professionals other than nurses, to clarify the reasons for not using assessment scales, and to provide education for pain assessors.
