Abstract 「要旨」目的は, 急性期病院病棟看護師による在宅訪問後の認識と退院支援の行動の変化を明確にすることである. 急性期病院病棟看護師12名に半構成的面接を行い質的記述的に分析した. 結果, 在宅訪問に対し【訪問時のスキルが未熟である】【在宅訪問が安心感を与える】【受け持ち看護師による在宅訪問が有効である】【住み慣れた家での充実した療養生活が見える】【患者と家族の関係や家族からの支援状況がわかる】【文化や言葉の違いによる家族の行動がわかる】【訪問看護師の視点と指導方法がわかる】等と認識していた. 在宅訪問後の退院支援では【個別性に配慮した具体的な指導ができる】【退院後の生活を継続的に捉え退院指導に活かす】【患者・家族の思いを尊重した支援を行う】【地域のスタッフと情報共有し積極的に連携を行う】等としていた. 在宅訪問のあり方には課題があるが, 学びもありその経験は次の退院支援に繋ぐ行動変化をもたらすことが示唆された.

Title Changes in Perceptions and Discharge Support Behavior of Ward Nurses in Acute Care Hospitals after Home Visits

Abstract [Abstract] We aimed to clarify changes in perceptions and discharge support behavior by ward nurses in acute care hospital after home visits. Semi-structured interviews were conducted with 12 nurses in an acute care hospital ward, and they were analyzed qualitatively and descriptively. The results showed that the nurses recognized the following in response to home visits : [being inexperienced in the skills needed for home visits], [home visits providing reassurance], [the effectiveness of home visits conducted by the primary nurse], [gaining insight into the patient's fulfilling life in their familiar home environment], [understanding the relationship between patients and their families and the support provided by families], [understanding the behavior of families influenced by cultural and linguistic differences], and [gaining a perspective on home care nurses' views and teaching methods]. After the home visits, the nurses changed their behavior as follows : [providing individualized and specific guidance], [considering the patient's life after discharge and apply it to discharge guidance], [supporting patients and families by respecting their wishes], and [sharing information proactively and collaborating with community staff]. Although there are challenges in the approach to home visits, it was suggested that the experience promotes behavioral changes that will lead to the future discharge support.
