Abstract 「要旨」 背景と目的:乳がん患者の希望に関する研究は少ない. 成人前の子どもを育てている初発乳がん患者が, 退院前に抱く希望を明らかにし看護支援の方法を検討する. 対象と方法:研究参加に同意の得られた18歳以下の子どもを持つ20〜40歳代の初発乳がん患者10名で, 手術後3日以降に半構成的面接および看護記録からデータを収集した. 質的帰納的手法により患者の希望に関する言語をコード化し, 類似性に沿ってサブカテゴリ・カテゴリ化へと抽象化した. 結果:成人前の子どもを育てている初発乳がん患者が, 退院前に抱く希望は【治癒】【自己実現】【自分と周囲の成長】の3つであることが明らかになった. 【治癒】は2つのサブカテゴリ『治療に専念』『健康の増進』から形成された. 【自己実現】には8つのサブカテゴリ『次子妊娠』『仕事が生きがい』『子どもとのふれあい』『母親役割の遂行』『自分らしく生きる』『社会の中で生きている』『今までのように生活』『将来の生活設計を立てる』から形成された. 【自分と周囲の成長】は4つのサブカテゴリ『子どもの成長」『夫の成長』『自分の成長』『家族関係の発展』から形成された. 結語:子育て中の乳がん患者の看護支援は, 今までと同じごく普通の生活をしたいという希望を実現するため, 先ず治療に専念出来るような環境調整と健康維持のための具体的な方法を指導することが必要であると示唆された. Title Hopes of Breast Cancer Patients who Have Children

Abstract [Background and Purpose] There are not many reports on the hopes of breast cancer patients. The purpose of this study is to clarify the hopes that primary breast cancer patients raising underage children have before discharge and to investigate nursing support for them. [Subjects and Methods] The subjects were 10 primary breast cancer patients aged 20-49 years having children under 18 who agreed to participate in the study. Data were collected from semi-structured interviews and nursing records on or after postoperative day 3. Using a qualitative inductive method, patients' words concerning hope were coded, and then the codes were abstracted into categories and subcategories according to similarity. [Results] Three hopes, “to be cured”, “self-realization” and “growth of themselves and people around them”, were revealed as the hopes that primary breast cancer patients raising children under 18 had before discharge. “To be cured” consisted of 2 subcategories, “to devote themselves to treatment” and “health promotion”. “Self-realization” consisted of 8 subcategories, “next pregnancy”, “work as the meaning of life”, “rapport with children”, “fulfillment of the mother's role”, “to live by their own values", “to live in society”, “to live as before” and “to plan their future life”. “Growth of themselves and people around them” consisted of 4 subcategories, “growth of children”, “growth of the husband”, “growth of themselves” and “development of family relationships”. [Conclusion] Our findings suggested that it is necessary in nursing support for breast cancer patients raising children to teach concrete methods for environmental adjustment and health maintenance, so that patients can first devote themselves to treatment as the first stage to realize their hope to live normal lives as before.
