| Abstract | 「要旨」 本研究は在宅療養高齢者のアドバンス・ケア・プランニング(ACP)の課題を訪問看護師の視点から明らかにすることを目的とする. 対象者は全国の訪問看護事業所1100件に勤務する看護師で, 郵送法による質問紙調査を実施した(回収率23.9%). そのうち, がん以外の疾病を有する在宅療養高齢者のACPの課題についての自由記載117名分を内容分析した. その結果, 【利用者自身による意思決定が困難】【家族が将来を見据えることができない】【利用者や家族が「いずれ訪れる死」を見据えられない】【家族の意向が優先される】【訪問看護師が意思決定に自信を持って関わることができない】【在宅ケアサービス提供者が多職種間及び利用者や家族と情報を共有していない】という問題, 今後の課題として【利用者の意思を引き出し, 家族も含めた話し合いを日頃より行う】【ACPが社会全体に普及し, 多職種間ならびに利用者や家族との話し合いができる】が導き出された. |

| Abstract | [ABSTRACT] The purpose of this study is to clarify the issues of advance care planning of the home care elderly from the visiting nurse's point of view. The subjects are nurses working at 1100 home-visit nursing establishment. A questionnaire survey was conducted by mail. The qualitative data of 117 subjects who described the issues of ACP of the home care elderly with non-cancer diseases was analyzed for consent. This study identified six categories of the problems of users, their families and home care service providers: "difficulties in making decisions by users themselves", "inability of families to look into the future", "inability of users and their families to accept death that will visit them sometime soon", "giving priority to family intentions", "inability of visiting nurses to engage in decision making with confidence", and "not sharing information among home care service providers and with users and their families". Furthermore, we identified two categories of future tasks: "drawing out the intentions of users and having regular discussions including the family" and "spreading the ACP throughout society and discussing between mufti-occupations and with users and families". |
