| Abstract | 「要旨」本研究は, 熟練看護師が, 終末期がん患者と家族間の対話をどのように支援しているのかを内容分析で明らかにすることを目的とした. 対象者は, 一般病棟で5年以上のがん看護の臨床経験があり, 終末期がん患者と家族間の対話支援経験を有する看護師とした. 対象者10名に半構造化面接を行い, 得られたデータを内容分析した. 対象者の看護師経験年数は, 平均21.9年であった. 対話支援として【患者と家族と何でも話せるような信頼関係を築く】【表立って現れていない患者と家族の本当の思いを確認する】【家族なりの思いの伝え方を尊重しながら介入の必要性をアセスメントする】【患者と家族が向き合えるよう対話の環境を調整する】【患者と家族の思いの言語化を促進する】の5カテゴリーが抽出された. 看護師は, 思いを伝えられるよう対話の流れを作り, 終末期がん患者と家族が自身の言葉で思いを語れるような支援を行っていると明らかになった. |

| Abstract | [ABSTRACT] [Objective] : This study aimed to clarify how expert nurses support dialogue between patients with cancer near the end-of-life and their families. [Methods] : The study participants were nurses who had more than 5 years of clinical experience in cancer nursing in a general ward, who had experience in supporting dialogue between patients with terminal cancer and their families. Data collected through semi-structured interviews were analyzed qualitatively and inductively using content analysis. [Results] : All nurses were aged between 30 and 60 years and had 15 to 30 years of experience as a nurse (mean, 21.9 years). The practices of expert nurses to support dialogue between patients with terminal cancer and their families were converged into the following five categories : Building a relationship of trust that allows me to talk with patients and their families, Confirming the true unsurfaced feelings of patients and their families, Assessing the need for intervention while respecting the way the family feels, Adjusting the environment for dialogue so that the patient and his/her family can face each other, and facilitating the verbalization of patients and their families thoughts. As all the study participants were expert nurses with abundant experience in providing end-of-life care, dialogue support was provided based on not only the clinical experience of each nurse, but also the experience of caring for the nurse's own family, which allowed both patients and their families to achieve satisfaction. Starting with helping patients and their families to notice their existence, the nurses adjusted the environment for dialogue to create a dialogue flow that enabled patients and their families to convey their thoughts. [Conclusion] : This made it clear that expert nurses provide dialogue support so that patients with terminal cancer and their families can speak in their own words. |
