Title NICUに入院する早産児を持つ母親への母乳育児支援効果に関する文献検討 - ランダム化比較試験に着目して -

Abstract 「要旨」研究目的は, NICU入院児を持つ母親への効果的な母乳育児支援について, ランダム化比較試験に限定した文献検討により明らかにすることである. 「NICU」「breast milk」「nursing」「母乳」などのキーワードで検索した. 11件の文献が抽出され, ハンドサーチによって5件を加え, 計16件を分析対象とした. 乳房の温罨法, 電動搾乳ポンプ, 授乳中の音楽療法, ガラクタゴーグの服用は搾乳量を増加させ, 非栄養的吸啜や両親による早期介入は, 完全母乳育児や経口栄養への移行期間を短縮させた. また, 授乳前の経口刺激, コントロールバブル式哺乳瓶用乳頭の使用は, 母乳育児率を上昇させ, カンガルー・マザー・ケアは母乳摂取する児の割合を増加させるなど, 母乳育児支援に有効であることが示唆された.

Abstract [ABSTRACT] The aim of this literature review is to clarify effective breastfeeding support for mothers of preterm infants, focusing on randomised controlled trials. We searched for randomised controlled trials of preterm infants or mothers of preterm infants from 2010 to 2022, using the keywords 'NICU', 'breast milk' and 'breastfeeding', and 11 references. In addition, five key foreign references were handsearched, and a total of 16 references were analysed. Nine effective interventions emerged: warm breast compresses, use of electric breast pumps, music therapy during breastfeeding, and galactagogue increased milking volume. Non-nutritive sucking (NNS), early intervention programme by parents shortened time for transition to full breastfeeding. Oral stimulation programme before breastfeeding, use of bottle teats with control bladder, and kangaroo mother care increased breast-feeding rate.
