Abstract | 「はじめに」近年, 脳卒中患者において早期離床に関する報告が多くなされるようになった. 脳卒中は脳梗塞, 脳内出血, くも膜下出血に分類されるが, それぞれの病態は大きく異なっており, リスク管理や離床の進め方が異なることは容易に想像がつく. 各病態に応じた適切なリスク管理と速やかな離床の開始は不活動に伴う合併症の予防や機能回復に寄与すると考えられる. 本稿では, 急性期脳卒中におけるリスク管理と早期離床について述べる. 「1. 脳卒中の病型分類」脳卒中とは脳の血管が詰まる虚血性脳卒中(脳梗塞)と血管が破れて出血する出血性脳卒中(脳出血)に分類される. 脳梗塞はThe Trial of org 10172 in acute stroke treatment(TOAST)分類によりアテローム血栓性脳梗塞, ラクナ梗塞, 心原性脳塞栓症, 他の原因による脳梗塞, 原因不明の脳梗塞に分けられ, TOAST分類には該当しないが, Branch Atheromatous Disease(BAD)という病型も存在する. |
