| Abstract | 「はじめに(抗凝固薬・抗血栓療法の変遷)」超高齢時代を迎え, 冠動脈疾患や脳血管疾患, 末梢動脈疾患など, 動脈硬化疾患による心血管イベントは, 悪性腫瘍と並んで死亡原因の上位を占めている. 抗血栓療法は抗血小板療法と抗凝固療法に分けられる. 抗血小板療法は, 抗血小板薬を用いて血小板の凝集を抑制するもので, 主として動脈系を中心としたアテローム血栓症の予防に有効とされている. 抗血小板薬としては低用量アスピリンが使われてきたが, 冠動脈病変をステントによって治療するようになり, アスピリンとチエノピリジンなどのADPアンタゴニスト(P2Y12受容体阻害薬)の2剤併用療法(dual antiplatelet therapy:以下, DAPT)が主流となった. その後, アテローム血栓症の急性期イベント予防にもDAPTが行われるようになり, 脳神経領域や末梢動脈領域など幅広く用いられるようになった. |
