| Abstract | 幻肢痛は失った四肢の部位に痛みが生じる現象であり, 不快な症状を伴うため患者のquality of lifeを著しく低下させる. しかし, 幻肢痛に対して有効な薬物療法が確立していないのが現状である. 70歳代女性. 右下腿の痛みのコントロールが不良となったため, 右下腿切断術施行. 右下腿切断の手術4日後に幻肢痛が出現し, 10日後に幻肢痛の増悪が認められた. 右下腿切断術前より使用していたオキシコドン徐放錠, アセトアミノフェン錠, メキシレチンカプセル, デュロキセチンカプセルでは幻肢痛は改善しなかった. そのため, 幻肢痛に対して抑肝散を投与したところ, 3日後には軽度改善, 4日後には消失した. 幻肢痛の消失後も, 幻肢痛は認められなかったため, メキシレチンカプセルが中止となった. 本症例より, 幻肢痛に対して抑肝散が有効である可能性が示唆され, 抑肝散が幻肢痛に対する薬物療法の1つになると考えられる. |
