| Abstract | 近年, 分子標的薬, 免疫チェックポイント阻害薬が数多く薬事承認されるなか, B型肝炎ウイルス (以下, HBV) 再活性化に関して添付文書上注意喚起のない抗がん薬も多く, 臨床現場では, その取り扱いが課題となっている. 今回我々は, 添付文書上注意喚起のない抗がん薬投与後にHBVが再活性化した症例を3例経験したので報告する. 症例1は, 非活動性キャリアの肺がん患者であり, クリゾチニブ投与中に, 症例2は, 既往感染の肺がん患者であり, ペムブロリズマブを投与後に, 症例3は, 既往感染の直腸がん患者であり, オキサリプラチン+フルオロウラシル+レボホリナート併用療法 (mFOLFOX6) を投与中にHBVが再活性化した. 3例とも適切にエンテカビルを使用したため, HBV-DNAは陰性化し重篤な肝障害を引き起こさなかった. このことから, 添付文書上注意喚起がない抗がん薬においてもHBVスクリーニングは重要であると考える. |
