| Abstract | 要旨:近年, 骨・軟部組織の感染制御における持続局所抗菌薬灌流療法(continuous local antibiotics perfusion:CLAP)の有効性を示す報告が増加している. 当院でCLAPを施行した36例の安全性と有効性を調査した. 安全性の評価は治療前後の腎機能, ゲンタマイシン(GM)最大血中濃度, 第8脳神経障害の有無, 骨癒合不全を調査し, 有効性の評価は感染制御率, 感染鎮静化までの日数を調査した. 治療前後で腎機能には有意な変動はなく, GM最大血中濃度は平均0.75μg/mLであった. 腎機能障害は1例に認めた. 第8脳神経障害は認めず, 骨癒合不全を2例に認めた. 感染制御率は予防群86%(12/14例), 治療群100%(22/22例)であった. 治療群での感染の鎮静化までの期間は平均11.5日であった. 当院における36例のCLAP施行は, 高い安全性と有効性を示した. |

| Abstract | In recent years, the effectiveness of continuous local antibiotics perfusion (CLAP) for bone and soft tissue infections has been increasingly reported. This study aimed to investigate the safety and efficacy of CLAP in 36 cases conducted at our institution. Safety evaluation included monitoring renal function, maximum gentamicin (GM) blood concentration, ototoxicity presence, and incidence of non-union. Efficacy assessment involved the remission rate of infections and the duration required for infection control. No significant differences in renal function were observed before and after CLAP, and the average maximum GM blood concentration was 0.75 μg/mL. Drug-related renal dysfunction was observed in one case, and no incidence of ototoxicity or other adverse events was detected. Two cases developed non-union. The infection remission rate was 86% (12/14 cases) in the prophylactic group and 100% (22/22 cases) in the treatment group. The average duration until infection remission in the treatment group was 11.5 days. These findings from the 36 cases of CLAP performed at our hospital demonstrate a high level of safety and efficacy. |
