| Abstract | 「要旨」本邦小児において, 好酸球性胃腸炎に伴う十二指腸潰瘍 (Eosinophilic Gastroenteritis-associated Duodenal Ulcer : EDU) の症例報告が散見されるが, 十二指腸潰瘍全体における割合は不明である. 群馬大学医学部附属病院小児科で2010年1月1日から2021年1月31日までに上部消化管内視鏡検査を行い, 十二指腸潰瘍と診断された患者を抽出した. 診療録を用いて原因, 診断時年齢, 性別, 初発時症状, 基礎疾患, 薬剤投与歴 (ステロイド, NSAIDs) , 内視鏡所見, 末梢血好酸球数, 治療内容, 再発の有無を検討した. 十二指腸潰瘍と診断された症例は14例で, 原因としては好酸球性胃腸炎が最も多く7例 (50%) を占めた. EDU群では全例再発していたのに対し, 非EDU群では1例に留まった (P<0.05) . その他の項目に関しては有意差を認めなかった. EDUの臨床像に特異的所見はなく, 問診や血液検査等からをEDUを予測することは困難であった. 十二指腸潰瘍を認めた際には好酸球性胃腸炎を常に念頭に置き, 内視鏡検査を検討することが重要である. |

| Abstract | Few reports on the frequency of eosinophilic gastrointestinal disorders as the cause of duodenal ulcers among children have been published to date. A single-center retrospective analysis was performed to clarify the prevalence and clinical features of eosinophilic duodenal ulcer (EDU) among Japanese children. Data on endoscopic diagnosis, age at onset, sex, medication history, laboratory findings, and clinical course of patients with duodenal ulcers diagnosed under esophagogastroduodenoscopy were reviewed using medical records at the Gunma University Hospital between January 2020 and January 2021. All patients in the EDU group experienced a recurrence of duodenal ulcers, whereas only one patient in the non-EDU group experienced recurrence (P<0.05) .The clinical course of EDU is nonspecific and difficult to predict based on medical history and blood test results. Therefore, when a duodenal ulcer is detected, it is important to consider eosinophilic gastroenteritis as a differential diagnosis and to perform an endoscopic examination. |
