| Abstract | 「要旨」 オスラー病は, 鼻出血, 皮膚粘膜毛細血管拡張症, 内臓血管病変(肺・脳・肝・消化管等), 家族歴の4項目中3項目該当で診断可能な常染色体優性遺伝形式をとる全身性血管疾患である. 多彩な病像と低い認知度のため多くは未診断で, 重篤なイベントにより初めて診断されることが多い. 患者は45歳女性, 鼻出血と皮膚粘膜病変は軽微であったが, 奇異性脳塞栓を契機に肺動静脈瘻と肝臓動静脈奇形が確認されオスラー病の診断に至り塞栓術を行った. 血縁者のスクリーニング検査で18歳の長女に肺動静脈奇形が確認された. 合併症回避のため, 菌血症を伴う医療行為時の抗生剤投与や日常生活の注意, 妊娠出産への備え等について指導を行った. 本疾患の合併症予防には, 医療者・患者家族・一般住民への啓発による診断率の向上と, 家族への積極的な介入が重要である. |
