| Abstract | 要旨:脳卒中後うつ(Post Stroke Depression;以下, PSD)に対し, 早期に抗うつ薬の開始が勧められているが, リハビリテーションにおいて有効な介入方法は体系化されていない. 脳卒中発症から半年以内でうつ症状が多くみられ, 日常生活動作や身体能力の回復に重大な影響を及ぼすと報告されており, 脳卒中発症早期から運動機能のみならず, 精神心理症状も含めた包括的な視点で介入していく必要がある. そこで, 生活行為向上マネジメント(Management tool for daily life performance;以下, MTDLP)を活用し, 希望する生活行為を焦点化し, 機能の説明, 予後の提示, 目標に対しての道筋を示し, 事例・家族・他職種と問題点や目標を共有し介入した. その結果, うつ症状の改善を認め, 希望する生活行為を達成することができた. 本介入により, PSDの対象者に対し, MTDLPを活用し介入することの有効性が示唆された. しかし, 本報告では一事例と対象者が少なく, 今後も事例を重ね有効性を検証していく必要がある. |
