| Abstract | 「要旨」当院での四肢長管骨の転移性骨腫瘍に対する治療状況とがんのリハビリテーション(以下, がんリハ)の効果について検討して報告する. 2013年1月〜2019年2月までの期間に四肢長管骨の転移性骨腫瘍に対して手術を行った症例を対象として, 原発部位と新片桐スコア, Mirelsスコア, 手術方法, リハの介入効果について検討を行った. 対象となったのは, 20例27肢, 男性8例, 女性12例, 平均年齢は73.7歳であった. 原発部位は, 多発性骨髄腫7例, 乳がん5例, 腎細胞がん5例, 前立腺がん, 肝細胞がん, 直腸がんがそれぞれ1例であった. 転移部位は, 大腿骨20肢, 脛骨3肢, 上腕骨4肢であった. 手術適応はMirelsスコアと予後を考慮して決定し, 術式は27肢すべて髄内釘固定であった. 術後がんリハ介入を行い, Performance Statusは改善(p=0.003), Barthel Indexは上昇(p=0.015)した. 四肢転移性骨腫瘍の病的骨折と切迫骨折に対しては, 早期の手術とがんリハ介入を行い, ADLを改善させることが重要である. |
