| Abstract | 「要旨」【目的】近年, 骨・軟部組織感染の制御において, 持続局所抗菌薬灌流療法としてiMAP, iSAPの有用性の報告が散見される. 今回我々は, iMAP, iSAPの安全性について検討を行った. 【対象と方法】開放骨折, 広範囲軟部組織損傷に対して感染の制御目的にiMAP, iSAPを行った16例18件を対象とした. 男性10例, 女性6例, 平均年齢は49.8(19〜93)歳であった. iMAP:4件, iSAP:11件, iMAP/iSAP:3件で, ゲンタマイシン硫酸塩(GM)を40〜240mg/day, 平均11(4〜21)日使用した. 治療前後の腎機能(BUN, Cr, eGFR), GM最大血中濃度, 第8脳神経障害の有無, 骨癒合不全の有無を調査した. 【結果】GM最大血中濃度は平均0.56(0.2〜1.4)μg/mlであり, 有意な腎機能障害, 第8脳神経障害の発生は認めず, 骨癒合不全は2例に認めたが, いずれも固定力不足による影響が明らかであった. 【結論】GMを用いたiMAP, iSAPは骨・軟部組織の感染の制御に有用な治療の選択肢であるが, 経静脈投与同様, 安全性に留意することが必要であると考えられた. |
