| Abstract | 「要旨」上腕骨大結節骨折を伴う上腕骨近位端骨折における大結節骨片の転位の有無による特徴について調査した. 上腕骨外科頚と大結節に骨折のある上腕骨近位端骨折に対して手術を行い, 術前に3DCTを施行した95例95肩(平均年齢71.1歳, 男性24肩, 女性71肩)を対象とした. 大結節骨折の転位の有無で, 転位なし群30肩, 転位あり群65肩に分類した. 転位なし群では大結節骨折範囲がfacetの1か所であったのが8肩, 2か所が20肩, 3か所が2肩であったのに対して, 転位あり群では1か所が0肩, 2か所が10肩, 3か所が55肩であり, 2群間で有意差を認めた. 大結節骨折を伴う上腕骨近位端骨折において, 大結節骨折の転位がないものの多くは骨折部位がfacetの1か所か2か所に留まっていたが, 転位しているものは骨折部位がsuperior facet, middle facet, inferior facetすべてに及んでいるものが多く, facetの1か所に留まるものは認めなかった. |
