| Abstract | 「要旨」大腿骨近位部骨折における入院時の栄養状態と術後せん妄の関係性を検討する. 対象は2020年1月〜2021年12月に当院で手術を施行した大腿骨近位部骨折206例とした. 栄養状態の評価として, geriatric nutritional risk index(GNRI)を算出し, せん妄への関与を後ろ向きに検討した. せん妄に対するGNRIの影響をロジスティック回帰分析により検討した. 交絡因子として年齢, ASA, 合併症(DM, 心疾患, 神経疾患, 呼吸器疾患), 性別, 骨折型, 術式, 待機日数, 認知症の有無, 睡眠薬の使用, 麻酔方法を調査した. GNRI低値はせん妄に影響する因子であった(オッズ比: 1.05, 95%信頼区間: 1.05〜1.10, p=0.008). ROC解析ではGNRI 95.5がカットオフ値であった. |
